چکیده
Pulmonary vein isolation (PVI) is the cornerstone treatment for management of atrial fibrillation (AF). However, patients with persistent AF often require additional ablation strategies to improve clinical outcomes and freedom from AF. The CONVERGENT procedure which combined epicardial endocardial approach ablation, offers improved efficacy in who have longstanding refractory prior ablation. We describe our experience establish convergent undergone at least one retrospectively analyzed 26 underwent procedure. All surgical posterior wall left atrium through a sub-xiphoid approach. This was followed by radiofrequency next day. Patients were an interval 1, 3, 6 months 1 year via static electrocardiograms or implanted devices assess outcomes. Baseline patient characteristics included average age 62± 9 years, 80% males. mean duration 2.75 ± 1.7 years ventricle ejection fraction 60 10.5%. Sinus rhythm achieved intraoperatively all 18 (69.2%) remained sinus follow up. Recurrence tachyarrhythmia (AF flutter) occurred 8 (30.7%) without use antiarrhythmic drugs. alone (23%) patients, 5 (19.2%) required repeat Anti-arrhythmic drugs discontinued 16 (61.5%) patients. Complication rate perioperative period 7.6%. hospital stay 2 days. Convergent effective strategy achieving despite Individualization therapy assessment risks benefits are important consider.

The Cox-Maze Procedure for Lone Atrial Fibrillation: A Single-Center Experience Over 2 Decades
This study reviews our experience in the surgical treatment of atrial fibrillation (AF) during the past 2 decades in 212 consecutive patients with lone AF. Freedom from AF in the original Cox-Maze III procedure was 93%. However, the procedure was difficult to perform and had a 10% rate of major complications. By using radiofrequency bipolar clamp technology to replace surgical incisions with tr...
متن کاملThe cox-maze procedure for lone atrial fibrillation: a single-center experience over 2 decades.
BACKGROUND The Cox-Maze procedure (CMP) has achieved high success rates in the therapy of atrial fibrillation (AF) while becoming progressively less invasive. This report evaluates our experience with the CMP in the treatment of lone AF over 2 decades and compares the original cut-and-sew CMP-III to the ablation-assisted CMP-IV, which uses bipolar radiofrequency and cryoenergy to create the ori...

Outcome of Circumferential Pulmonary Vein Isolation for Ablation of Atrial Fibrillation: A Single Center Experience.
BACKGROUND Atrial fibrillation (AF) is the most common heart rhythm disorder in adults. Currently, use of the circumferential pulmonary vein isolation (CPVI) technique is the cornerstone of ablation therapy for paroxysmal atrial fibrillation. In this report, we described our ablation strategy and outcomes when treating a limited number of AF patients. METHODS This study enrolled patients with...
